We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our night shift team at Meadow Falls of Madison, a resort-style senior living community offering a range of care options, including Assisted Living, Memory Care, Adult Day, and Respite Care. Our team is committed to providing exceptional care and services to our residents, and we are searching for a dedicated and caring LPN to help us achieve this mission.

**Responsibilities:**

* Provide high-quality, individualized care to our residents, including administering medication, performing wound care, and assisting with daily living activities
* Develop and implement individualized care plans, in collaboration with the interdisciplinary care team
* Monitor and report changes in resident condition to the nursing staff and healthcare providers
* Participate in resident assessments, care planning, and coordination of services with other healthcare professionals
* Maintain accurate and detailed records of resident care, including medication administration and treatment plans
* Provide emotional support and companionship to residents and their families
* Collaborate with the interdisciplinary care team to ensure seamless transitions and care coordination

**Requirements:**

* Current Licensed Practical Nurse (LPN) license in the State of Ohio
* High school diploma or equivalent required
* Experience in long-term care or gerontological nursing is preferred
