Jethro Health is looking for experience Home Health Agency LPN to see patients in the North Shore area: Lynn, Peabody, Nahant, Revere, Winthrop, East Boston, Chelsea, Salem, Swampscott, Wakefield, Reading, Woburn, Saugus, Revere.

We are seeking a compassionate and dedicated LPN to join our Home Health team. The ideal candidate will provide quality care to patients in the comfort of their homes in the Lynn area, ensuring their well-being and promoting independence.

This is a part-time position, with potential to become Full-Time.

 

Responsibilities

  • Conduct thorough assessments of patients' health status and needs in their home environment.

  • Develop and implement individualized care plans based on patient assessments and medical history.

  • Provide education and support to patients and their families regarding health conditions, medications, and self-care techniques.

  • Assist with discharge planning from hospitals or rehabilitation facilities, ensuring a smooth transition back to home care.

  • Monitor patient progress and adjust care plans as necessary to meet evolving health needs.

  • Collaborate with interdisciplinary teams, including physicians, social workers, and therapists, to coordinate comprehensive care.

  • Document all patient interactions accurately in compliance with ICD-10 coding standards.

  • Offer health coaching to empower patients in managing chronic conditions effectively.

  • Provide specialized services such as esthetic laser treatment when applicable.

  • Support family planning initiatives and educate patients on related health topics.

 

Skills

  • Strong clinical skills with a focus on acute care and senior care practices.

  • Excellent communication skills for effective patient education and collaboration with healthcare teams.

  • Proficient in discharge planning processes and the ability to navigate complex healthcare systems.

  • Knowledge of ICD-10 coding for accurate documentation of patient encounters.

  • Experience working in a teaching hospital environment is preferred but not required.

  • Ability to provide compassionate care while maintaining professional boundaries.

  • Strong organizational skills to manage multiple patient cases effectively.

  • Commitment to ongoing professional development and staying current with best practices in nursing care.
