**About the Role:**

As a Weekend LPN, you will provide direct patient care, administer medications, and assist with daily living activities in our state-of-the-art facility. You will work closely with our interdisciplinary team to develop and implement individualized care plans, ensuring that our patients receive the highest level of care possible.

**Responsibilities:**

* Provide direct patient care, including vital sign assessments, wound care, and medication administration
* Assist with daily living activities such as bathing, dressing, and grooming
* Maintain accurate and detailed patient records
* Collaborate with the interdisciplinary team to develop and implement individualized care plans
* Provide education and support to patients and families
* Monitor and report any changes in patient condition to the nurse manager or physician

**Requirements:**

* Current LPN license in the state of Massachusetts
* Minimum 1 year of experience in a healthcare setting
* Ability to work a variety of shifts, including weekends and holidays
