LPN - Licensed Practical Nurse

The Licensed Practical Nurse (LPN) works under the direction of the Director of Nursing (DON) to provide quality, holistic nursing care in accordance with nursing practice standards and health care regulations that govern the practice of professional nursing. LPNs lead and direct licensed and non-professional staff in the delivery of direct resident care and support functions as assigned.

Key Responsibilities

• Perform and document patient assessments, including physical, psychosocial, ethnic, cultural, and spiritual needs.

• Develop, implement, and evaluate individualized nursing care plans in collaboration with the patient and care team.

• Administer medications and treatments, and provide patient education.

• Monitor patient status, identify significant changes, and respond quickly to emergencies.

• Delegate tasks and supervise nonprofessional staff.

• Adhere to all hospital policies, procedures, and safety standards, including infection control.

• Maintain clear and effective communication with patients, families, and the healthcare team.

• Complete shift duties efficiently and provide accurate patient reports.

• Participate in peer review and ongoing professional development.

Qualifications & Requirements

• Active, Non-Probationary LPN License

• Active, Class-Instructed CPR Certification

• Background Screening, Licensure Verification (if applicable), Reference Check, Drug Testing

• Employee Health Screening (Conducted after Hire)

• Ability to Move, Lift and Transfer a minimum of 50 Pounds
