About the Role:

As a Pediatric Home Health LPN/RN with Suma Home Care, your primary responsibilities will include:

• Providing skilled nursing care to pediatric patients in their homes, including wound care, medication management, and IV administration

• Collaborating with patients, families, and healthcare teams to develop and implement individualized care plans

• Conducting comprehensive assessments of patients' physical and emotional needs

• Educating patients and families on self-care techniques, medication management, and disease management

• Documenting patient information and communicating with healthcare teams in a timely and accurate manner

• Working closely with caregivers and home health aides to ensure seamless transitions and continuity of care

Ideal Candidate:

• Pediatric home care experience is preferred, but not required. We will consider qualified candidates who are eager to learn and grow with our team.

• Current LPN/RN license in the state of Pennsylvania with a strong understanding of pediatric nursing practices and standards.

• Excellent communication and interpersonal skills to build strong relationships with patients, families, and healthcare teams.

• Ability to work independently in a fast-paced environment while prioritizing patient safety and satisfaction.

• Basic computer skills, including proficiency in electronic medical records and communication systems.
