MDS Nurse - Thrive Rehabilitation of Pearland
Job Summary:
We are seeking an experienced and compassionate MDS Nurse to join our team at Thrive Rehabilitation of Pearland, a premier post-acute care facility in Pearland, Texas. As an MDS Nurse, you will play a critical role in ensuring the highest quality of care for our patients by conducting patient assessments, developing individualized care plans, and coordinating with the interdisciplinary team to achieve optimal outcomes.
Responsibilities:
Conducts patient assessments utilizing the Minimum Data Set (MDS) instrument to identify patient needs and develop individualized care plans
Coordinates with the interdisciplinary team to implement care plans and ensure seamless transitions
Maintains accurate and up-to-date medical records and documentation
Monitors patient progress and makes adjustments to care plans as needed
Collaborates with the care team to resolve any issues or concerns related to patient care
Participates in quality improvement initiatives to ensure optimal patient outcomes
Requirements:
Active TEXAS LVN/ RN LICENSE (Required!)
1 year of MDS experience (Required!)
1 year of Long Term Care experience (Required!)
Excellent customer service skills and a friendly demeanor
Ability to work in a fast-paced environment and prioritize tasks effectively
Strong communication and interpersonal skills
Ability to commute to our facility in Pearland, TX 77584 (Required)
