We are seeking a Medical Social Worker to serve patients in Newark, Ohio and surrounding communities. This is a field-based position that combines patient visits in homes and facilities with collaboration among an experienced interdisciplinary care team.
Schedule:
- Monday–Friday, 8:30 AM–5:00 PM
- Weekend and holiday rotation required.
- This position requires travel between patients homes and facilities located in Newark, Ohio and surrounding areas.
As a Hospice Medical Social Worker, you'll play a vital role in helping patients and families navigate the emotional, social, spiritual, and practical challenges that accompany serious illness and end-of-life care.
Key responsibilities include:
- Conduct comprehensive psychosocial assessments for hospice patients and families
- Provide counseling, emotional support, and crisis intervention as needed
- Assist families with advance care planning and community resource coordination
- Address cultural, spiritual, and psychosocial needs unique to each patient and family
- Support patients and loved ones through anticipatory grief and bereavement processes
- Collaborate with nurses, physicians, chaplains, and other members of the hospice care team
- Maintain accurate and timely clinical documentation
- Advocate for patient and family needs while promoting quality of life and comfort
Required Qualifications
- Bachelor's degree in Social Work
- Ohio Social Work license (LSW or LISW)
- Minimum of one year of social work experience in a healthcare setting
- Valid driver's license, reliable transportation, and acceptable driving record
- State-required automobile insurance coverage
- Strong computer and documentation skills
- Ability to travel throughout the service area, including during inclement weather
Preferred Qualifications
- Master's degree in Social Work (MSW)
- Hospice or palliative care experience
- CHP-SW or ACHP-SW certification
- CPR certification
- Notary designation or willingness to obtain within six months of hire
