Springfield Center for Living & Rehabilitation is seeking a dedicated and compassionate Part-Time Speech-Language Pathologist (SLP-CCC) to join our in-house rehabilitation team. This is an excellent opportunity to make a meaningful impact in a skilled nursing and long-term care setting while working alongside an experienced interdisciplinary team focused on exceptional resident outcomes.

Responsibilities

• Evaluate, develop, and implement individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders

• Provide evidence-based therapeutic interventions to maximize functional outcomes and quality of life

• Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and family members

• Complete timely and accurate documentation in Net Health and PCC

• Participate in care planning meetings and interdisciplinary team discussions

• Ensure compliance with all state, federal, and facility regulations

Qualifications

• Master's Degree in Speech-Language Pathology

• Current Vermont Speech-Language Pathology license

• Certificate of Clinical Competence (CCC-SLP) required

• Skilled nursing, rehabilitation, or long-term care experience preferred

• Strong clinical, communication, and organizational skills

• Commitment to providing high-quality, resident-centered care
