Responsibilities:

• Provide direct patient care in accordance with facility policies, physician orders, and safe nursing practices
• Assist licensed nurses with patient assessments, care planning, and treatment implementation
• Administer medications and treatments; monitor patients for side effects or adverse reactions
• Assist with vital signs, wound care, treatments, and skilled nursing interventions as ordered
• Observe, document, and report changes in patient condition to physicians and leadership
• Participate in admissions, discharges, and transfers
• Provide clear communication to patients, families, and healthcare team members
• Collaborate with the interdisciplinary care team to ensure comprehensive care
• Participate in care conferences and care plan updates
• Promote teamwork, professionalism, and effective communication
• Assist with state surveys and inspections as needed

Qualifications:

• Graduate of an accredited Licensed Practical Nurse (LPN) program
• Current, unencumbered State of Rhode Island LPN license
• Current CPR certification (ACLS preferred, not required)
• Long-term care or skilled nursing experience preferred (not required)
• Strong clinical judgment, communication, and interpersonal skills
• Ability to handle multiple priorities with professionalism and tact
