Position Summary
The Licensed Practical Nurse (LPN) is responsible for supporting the health, safety, and overall well-being of residents through the delivery of high-quality nursing care. This role combines clinical expertise with compassionate support, ensuring residents receive the medical attention, education, and encouragement needed to achieve their recovery goals.
Essential Duties & Responsibilities
- Administer medications, treatments, blood glucose monitoring, and other physician-prescribed care in accordance with state regulations and facility policies.
- Monitor resident health status and promptly identify changes in condition.
- Measure and record vital signs, including blood pressure, pulse, temperature, and weight.
- Maintain accurate and timely documentation in residents' medical records.
- Provide emotional support and therapeutic communication to residents.
- Educate residents regarding medications, treatments, and health-related concerns.
- Communicate abnormal findings, incidents, behavioral changes, lab results, and medication reactions to physicians and other members of the healthcare team.
- Respond effectively to medical emergencies and provide necessary nursing interventions.
- Participate in the development and implementation of resident care plans.
- Collaborate with interdisciplinary team members to ensure quality resident outcomes.
Qualifications
Required
- Current Licensed Practical Nurse (LPN) license in good standing in the State of Illinois.
- Current CPR Certification.
- Strong communication, documentation, and interpersonal skills.
- Commitment to providing compassionate, resident-centered care.
Preferred
- 2–3 years of nursing experience.
- Previous experience working in behavioral health, psychiatric, long-term care, or residential settings.
- Experience with care planning and interdisciplinary team collaboration.
Schedule Opportunities
We are currently hiring for the following 12-hour shifts:
- Day Shift: 10:00 AM – 10:30 PM
- Night Shift: 10:00 PM – 10:30 AM
Schedule includes every other weekend.
