Position Summary

The Licensed Practical Nurse (LPN) is responsible for supporting the health, safety, and overall well-being of residents through the delivery of high-quality nursing care. This role combines clinical expertise with compassionate support, ensuring residents receive the medical attention, education, and encouragement needed to achieve their recovery goals.

Essential Duties & Responsibilities

• Administer medications, treatments, blood glucose monitoring, and other physician-prescribed care in accordance with state regulations and facility policies.
• Monitor resident health status and promptly identify changes in condition.
• Measure and record vital signs, including blood pressure, pulse, temperature, and weight.
• Maintain accurate and timely documentation in residents' medical records.
• Provide emotional support and therapeutic communication to residents.
• Educate residents regarding medications, treatments, and health-related concerns.
• Communicate abnormal findings, incidents, behavioral changes, lab results, and medication reactions to physicians and other members of the healthcare team.
• Respond effectively to medical emergencies and provide necessary nursing interventions.
• Participate in the development and implementation of resident care plans.
• Collaborate with interdisciplinary team members to ensure quality resident outcomes.

Qualifications

Required

• Current Licensed Practical Nurse (LPN) license in good standing in the State of Illinois.
• Current CPR Certification.
• Strong communication, documentation, and interpersonal skills.
• Commitment to providing compassionate, resident-centered care.

Preferred

• 2–3 years of nursing experience.
• Previous experience working in behavioral health, psychiatric, long-term care, or residential settings.
• Experience with care planning and interdisciplinary team collaboration.

Schedule Opportunities

We are currently hiring for the following 12-hour shifts:

• Day Shift: 10:00 AM – 10:30 PM
• Night Shift: 10:00 PM – 10:30 AM

Schedule includes every other weekend.
