What You'll Do

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop individualized treatment plans based on each patient's goals and needs.
• Provide therapeutic exercises, balance training, gait training, and functional mobility interventions.
• Educate patients and caregivers on home exercise programs, safety, and fall prevention.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
• Complete timely and accurate electronic documentation.
• Monitor patient progress and adjust treatment plans as appropriate.

Qualifications

• Active Colorado Physical Therapist (PT) License.
• Graduate of an accredited Physical Therapy program.
• Excellent communication and patient education skills.
• Ability to work independently and manage a flexible schedule.
• Home Health experience is preferred but not required.
• Reliable transportation and valid driver's license.
