We are seeking an experienced and compassionate Care Coordinator – Registered Nurse (RN) to join our Primary Care team in Nome, Alaska. In this vital role, you will coordinate comprehensive patient care plans, serve as a liaison between patients, families, providers, and community resources, and help ensure patients receive timely, effective, and high-quality care.

The Care Coordinator RN plays a key role in promoting patient independence, improving health outcomes, and supporting interdisciplinary care management across a diverse patient population.

Key Responsibilities\n
    \n
  • Coordinate and oversee interdisciplinary patient care plans.
  • \n
  • Serve as a liaison between patients, families, healthcare providers, and community agencies.
  • \n
  • Promote patient independence through education on conditions, medications, and self-care strategies.
  • \n
  • Conduct ongoing follow-up and care management activities to ensure patients receive appropriate services.
  • \n
  • Monitor and support patient service plans, including reviews, assessments, and care coordination reporting.
  • \n
  • Maintain regular contact with patients and providers to evaluate progress and update care plans.
  • \n
  • Plan and organize specialty clinics, health promotion activities, and community outreach events.
  • \n
  • Provide relief RN coverage in the outpatient clinic as needed.
  • \n
  • Collaborate with multidisciplinary teams to deliver patient-centered care.
  • \n
QualificationsRequired Education\n
    \n
  • Associate Degree in Nursing required.
  • \n
  • Bachelor’s Degree in Nursing preferred.
  • \n
Required Experience\n
    \n
  • Minimum of 5 years of experience as a Registered Nurse.
  • \n
  • 8 years of RN experience preferred.
  • \n
  • Prior supervisory experience preferred, with at least 1 year of leadership experience.
  • \n
Licenses & Certifications\n
    \n
  • Current Alaska Registered Nurse (RN) License.
  • \n
  • Basic Life Support (BLS) certification.
  • \n
  • Advanced Cardiovascular Life Support (ACLS) certification within one year of hire.
  • \n
Knowledge, Skills & Abilities\n
    \n
  • Strong clinical nursing skills and judgment.
  • \n
  • Knowledge of care coordination, referrals, and patient documentation.
  • \n
  • Ability to manage complex and high-volume caseloads.
  • \n
  • Excellent communication, interpersonal, and patient advocacy skills.
  • \n
  • Ability to support patients and families during emotionally challenging situations.
  • \n
  • Strong organizational and time-management abilities.
  • \n
  • Commitment to confidentiality, teamwork, and patient-centered care.
