Responsibilities:
\n -Coordinate patient care plans and discharge needs throughout hospitalization.
\n -Conduct comprehensive assessments to identify barriers to discharge and continuity of care.
\n -Collaborate with physicians, nurses, social workers, and interdisciplinary teams to develop effective care plans.
\n -Facilitate timely discharges and appropriate transitions to post-acute care settings.
\n -Educate patients and families regarding treatment plans, resources, and discharge instructions.
\n -Advocate for patients while ensuring quality, cost-effective care delivery.
\n -Maintain accurate and timely clinical documentation.
\n -Support compliance with hospital policies, regulatory requirements, and quality standards.
\n -Facility Type: Acute Care Hospital

Minimum Qualifications:
\n -Active California Registered Nurse (RN) License
\n -Minimum of 1+ years of recent Acute Care Hospital Case Management experience
\n -Recent experience performing:
\n -Care Coordination
\n -Discharge Planning
\n -Utilization of community and post-acute resources
\n -Acute Care Hospital Case Management experience is required
