\n a copy of FL license
\n a copy of current ARRT card
\n a copy of Mammography registry certificate (if no date on it, need ARRT documentation showing registry date)
\n 8 initial hours of digital training
\n 8 initial hours of tomosynthesis training
\n 15 current mammography CME's
\n Initial 40 hours of training/ attestation
\n Current 200 mammography stats
