Pay Range: $56.29 - $59.10 per hour
Schedule: 40 hours per week (Monday - Friday, 8 am - 5 pm)
Contract Length: 13 weeks
Candidate Type: Local candidates only – travel stipend not available
Requirements:

- Must have a minimum of 1 year of home health experience with a Medicare-certified facility \n
- Qualified applicants MUST have WA license; candidates can start with WA and obtain OR license \n
- Valid RN license in the state of Washington \n
- Be willing to obtain Oregon licensure \n

Responsibilities:
\n As a healthcare professional in a home health setting, you will support patients in their homes, deliver personalized care, and coordinate with interdisciplinary teams to ensure quality outcomes.

- Provide comprehensive RN assessments and care plans in a Medicare-certified home health environment (RN) \n
- Utilize HCHB EMR system to document patient care accurately and efficiently, demonstrating proficiency in documentation skills \n
- Collaborate with physicians and caregivers to develop and implement patient-centered care strategies \n
