American Traveler is seeking an experienced Home Health RN with a minimum of 2 years of experience and OASIS documentation proficiency for a 13-week travel contract in Vermont.

- Home health setting serving a patient population with complex care needs including wound care, IV therapy, and symptom management \n
- Serves as Case Manager for assigned patients, managing caseloads and visit times in accordance with agency productivity standards \n
- High-tech skills required including IV use and maintenance, wound VAC, and complex wound care \n
- Uses McKesson for charting and documentation \n
- OASIS documentation is a core part of this role \n
- Schedule is Monday–Friday, 8am–4pm, with some on-call and weekend requirements \n

- Active RN license in Vermont (VT) or a compact RN license that is currently valid and in hand \n
- Minimum 2 years of RN experience required \n
- OASIS documentation experience required \n
- Proficiency with McKesson EMR \n
- Skills in IV therapy, wound VAC, and complex wound care required \n
- \n
- Conducts comprehensive patient and caregiver assessments using the nursing process to develop and implement individualized plans of care \n
- Collaborates with interdisciplinary team (IDT) members including physicians, LPNs, physical therapists, social workers, and other providers to coordinate patient care \n
- Participates in IDT meetings, case conferences, and monthly peer review audits \n
- Performs patient and caregiver education on topics including medication side effects, treatment options, discharge instructions, and reportable conditions \n
- Responsible for accurate and timely completion of agency, state, and federal forms including HHABN and Medicare Notice of Non-Coverage \n
- Identifies and communicates interdisciplinary patient needs in a timely manner to ensure continuity of care \n
- Coordinates safe patient discharges and/or transfers, ensuring all required documentation is completed per policy \n
