About the Role:
As an LPN, you will play a vital role in providing high-quality patient care in our nursing team. You will work nights, Monday through Sunday, supporting the health and well-being of our residents. We offer a 5000 sign on bonus for this role, as well as opportunities for professional growth and development.
Responsibilities:
• Provide direct patient care, including administering medications, performing treatments, and assisting with daily living activities
• Monitor and document patient conditions, identifying changes and reporting to the healthcare team
• Participate inInterdisciplinary care planning and collaborative goal-setting with the healthcare team
• Maintain accurate and complete records, adhering to HIPAA guidelines
• Provide exceptional customer service, building strong relationships with patients, families, and staff
• Collaborate with other healthcare professionals to ensure high-quality, patient-centered care
• Demonstrate a commitment to quality, safety, and regulatory compliance, adhering to all policies and procedures
Requirements:
• Current LPN licensure in the state of Pennsylvania
• Ability to work nights, Monday through Sunday
