Primary Care/Acute Care

ASAP - Dec. 31.

Locum Schedule with Lunch Times Included

M,T,TH 8-6p,

Wed 8-4p,

Fri 8-12p

Ages of children seen: 2 and up but there are not many kids at all. Mostly all adults

Onsite Dispensing: yes

DOT -yes

Occupational health? yes

Workers Comp? yes

Is women’s health required for temp/locum coverage? yes

Health center staffing model: 1:1 one provider one MA

2 years or more of FNP work experience

DEA

Prescribing license

Dispensing license

Clean WI license

Malpractice, lodging provided.

Mileage reimbursed.
