Social Worker

Key Responsibilities

• Conduct comprehensive psychosocial and mental health assessments.
• Develop, implement, and monitor individualized treatment plans.
• Provide individual and group counseling services.
• Perform crisis assessments, triage, and appropriate interventions.
• Track client progress and adjust treatment plans as needed.
• Coordinate care and make referrals to healthcare providers and community resources.
• Maintain accurate, timely, and confidential clinical documentation.
• Collaborate with interdisciplinary teams to provide integrated healthcare services.
• Support clients' overall mental health and wellness through ongoing care and education.

Qualifications

• Master's degree in Social Work (MSW) from an accredited program.
• ActiveNew York Licensed Clinical Social Worker (LCSW)license.
• 1–2 years of experience providing clinical services in the mental health field.
• Bilingual (English/Spanish) is a plus.
