Family Nurse Practitioner (APRN) – Primary Care
Position Type: Part-Time (Flexible 2-Day Work Week) with potential to be full time
Compensation: $65.00 – $70.00 per hour
Location: Canton, CT (On-site)
Schedule: Optional 2 days per week (e.g., 20 hours total, flexible daytime scheduling with no weekends, no holidays, and absolutely no on-call responsibilities).

We are growing and seeking a dedicated, patient-centered Family Nurse Practitioner (FNP) to join our growing clinical team in this part-time role.
Position Summary: The Family Nurse Practitioner will provide comprehensive primary care services across the lifespan, emphasizing preventative medicine, chronic disease management, wellness, and patient education. The ideal candidate values whole-person care, enjoys building long-term patient relationships, and thrives in a collaborative environment.
Responsibilities:
- Conduct comprehensive patient assessments and physical exams.
- Diagnose and treat acute and chronic medical conditions.
- Develop individualized treatment and wellness plans.
- Provide preventative care, annual wellness visits, and health screenings.
- Manage chronic conditions including hypertension, diabetes, obesity, and mental health conditions.
- Order, interpret, and review diagnostic tests and laboratory work.
- Prescribe medications and treatments in accordance with Connecticut regulations.
- Educate patients on lifestyle modifications and preventative health strategies.
- Collaborate with interdisciplinary providers to ensure continuity of care.
- Maintain accurate and timely electronic medical records (EMR).
Qualifications:
- Active Connecticut APRN license (or eligibility to obtain prior to start date).
- Board certification as a Family Nurse Practitioner (FNP-C or ANP/FNP equivalent).
- Experience: Minimum of 1–2 years of APRN clinical experience is preferred; however, new graduates with a strong, diverse RN background are encouraged to apply and will be considered.
- DEA registration and prescriptive authority.
- Strong clinical assessment and diagnostic skills.
- Genuine passion for holistic, root-cause, and patient-centered care.
- Bilingual English/Spanish is a plus.
