RN Care Manager | Full-Time | Day Shift

Job Summary

We are seeking an experienced RN Care Manager to coordinate patient care, discharge planning, and care transitions. You'll work closely with physicians, nurses, patients, and families to ensure quality care and positive patient outcomes.

Compensation

• Starting at $30.11+/hour (based on experience)
• Relocation Assistance: Up to $5,000

Responsibilities

• Coordinate patient care and discharge planning.
• Assess patient needs and develop care plans.
• Collaborate with physicians and healthcare teams.
• Ensure timely and appropriate care transitions.
• Maintain accurate documentation.

Requirements

• Associate Degree in Nursing (ADN) or Diploma RN.
• Active Kansas RN License.
• Minimum 2 years of acute care Med-Surg RN experience.
• BSN preferred.
