Schedule: Full-time (Flexible)

Location: Chicago, IL

Pay range: $40-$55/hour

Position Overview

A Home Health Physical Therapist in Chicago travels directly to patients' homes. They evaluate mobility and build personalized plans. They administer therapeutic exercises and educate patients on fall prevention.

Key Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients in your community

• Evaluate patient mobility, strength, and function to develop individualized treatment goals

• Implement effective rehabilitation and therapeutic exercise programs

• Educate patients and families on recovery plans and preventive care

• Maintain accurate and timely home health care documentation per Medicare and agency standards

Requirements

• Active physical therapy (PT) license in good standing

• BLS CPR certification

• Reliable transportation to see patients in their homes
  

Qualifications

• Active Illinois Physical Therapist (PT) license/certification required.

• Graduate of an accredited Physical Therapist program.

• Prior experience working as a Physical Therapist (PT) is required.

• Home health experience preferred, but not required.

• Current CPR certification.

• Strong communication, organizational, and time management skills.

• Ability to work independently in a home health setting.

• Valid driver's license and reliable transportation for travel throughout the Chicago service area.
