Job Description
Quality Healthcare Group Inc. is seeking a licensed Occupational Therapist to deliver skilled, functional, and patient-centered therapy services in home and residential environments. The OT will evaluate patients’ ability to perform activities of daily living (ADLs), design individualized treatment plans, and collaborate with interdisciplinary teams to improve independence, safety, and participation in meaningful activities.
This role is structured to be competitive with leading regional home health employers while maintaining manageable productivity expectations and strong clinical autonomy.
Key ResponsibilitiesClinical Evaluation & Treatment
Conduct comprehensive occupational therapy evaluations assessing ADLs, IADLs, cognition, upper-extremity function, coordination, vision/perception, sensory needs, and environmental barriers.
Develop measurable, function-based treatment plans aligned with physician orders and patient goals.
Provide skilled interventions including:
ADL/IADL retraining (dressing, bathing, toileting, feeding, meal prep)
upper-extremity strengthening and coordination training
fine motor and cognitive rehabilitation
adaptive equipment assessment and training
home safety and fall-prevention interventions
energy conservation and task modification strategies
Train patients, caregivers, and residential staff on safe techniques, adaptive tools, and home programs.
Home & Residential Care Coordination
Evaluate environmental risks and recommend modifications to improve safety and independence.
Collaborate with nurses, PTs, speech therapists, behavioral staff, and direct support professionals.
Support transitions from hospital or rehab to home/community living.
Participate in interdisciplinary case conferences and care planning.
Documentation & Compliance
Complete timely, accurate documentation supporting medical necessity and skilled need.
Maintain compliance with Medicare, Medicaid, CHC/MCO, and Pennsylvania DHS requirements.
Track progress toward functional goals using objective outcome measures.
Perform reassessments and discharge planning based on patient progress.
Master’s or Doctorate in Occupational Therapy from accredited program
Active Pennsylvania OT license in good standing
CPR certification
Valid driver’s license and reliable transportation for community visits
Ability to work independently in patient homes and residential settings
Experience in home health, geriatrics, neuro rehab, or IDD/TBI populations
Familiarity with Axxess or AloraPLUS
Experience in adaptive equipment prescription and environmental modification
Prior caregiver/staff training experience
Bilingual skills a plus
