Occupational Therapist

Our Lady of Consolation is committed to improving our patients' quality of life & autonomy through innovative rehabilitative services, and an exceptional level of client-centered care and attention. We are seeking a diligent and skilled Occupational Therapist to join our team in West Islip. 

Job responsibilities include:

• Provide occupational therapy to residents
• Develop and improve the skills needed for the daily functioning of residents
• Accurately completes and documents all treatment procedures and functional mobility tasks in a timely manner
• Establish a plan of care in accordance with patients’ needs and capabilities
• Apply interventions to maximize performance for daily living and instrumental activities
• Prepare documentation, progress notes, reports and discharge summaries as required

Qualifications Include:

• Valid NY OT License and Credentials
• SNF and Sub-Acute/Long-Term Care experience preferred
