Fifth Star Care Management is seeking a dedicated and compassionate Speech Language Pathologist (SLP) to join our IN-HOUSE therapy team! New Grads Welcome!

$10,000 sign-on and re-location bonus for Full-time position!

 

Responsibilities

  • Evaluate, diagnose, and develop individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders.

  • Deliver evidence-based therapy services in accordance with physician orders and facility policies.

  • Collaborate closely with an interdisciplinary team, including Physical Therapists, Occupational Therapists, Nurses, and Physicians, to ensure comprehensive care.

  • Educate patients, families, and caregivers on treatment goals, home programs, and progress.

  • Maintain accurate and timely clinical documentation in compliance with state, federal, and facility guidelines.

  • Participate in care plan meetings and contribute to discharge planning as needed.

Qualifications

  • Master’s Degree in Speech-Language Pathology from an accredited program.

  • Current State Licensure (or eligibility for licensure) as a Speech-Language Pathologist.

  • Current Certificate of Clinical Competence (CCC-SLP) from ASHA (CFY candidates welcome to apply).

  • Prior experience in a Skilled Nursing Facility or similar setting preferred, but not required.

  • Strong interpersonal, organizational, and communication skills.
