Job Overview

Deer Meadows Home Health is hiring a Speech Language Pathologist (SLP/CCC) for Home Health and Community-Based care. This role offers flexible Part-Time and PRN schedules and the opportunity to treat patients across home, ALF, IL, and personal care settings.

Ideal candidates have Home Health experience and are comfortable working independently while delivering high-quality, patient-centered care.

Key Responsibilities

• Evaluate speech, language, cognitive, and swallowing disorders

• Develop and implement individualized treatment plans

• Provide in-home and community-based speech therapy services

• Perform reassessments, progress notes, and discharge planning

• Educate patients, families, and caregivers

• Collaborate with physicians and interdisciplinary teams

• Complete documentation in Home Care Home Base (HCHB) EMR

• Manage caseload and schedule efficiently

Requirements

• Master’s or Doctorate in Speech-Language Pathology

• Active Pennsylvania SLP License

• CCC-SLP (ASHA) – Required

• Home Health experience – Required

• Experience with Home Care Home Base (HCHB) preferred

• Valid driver’s license & reliable transportation

• Strong communication, organization, and time management skills
