Job Summary

The Licensed Practical Nurse (LPN) provides direct nursing care to residents under the supervision of a Registered Nurse (RN) or physician. The LPN is responsible for delivering high-quality patient care, administering medications and treatments, monitoring resident conditions, documenting care, and collaborating with the interdisciplinary healthcare team to promote positive patient outcomes. The LPN ensures compassionate, safe, and efficient care while maintaining compliance with facility policies and regulatory standards.

Job Requirements

Education and Experience:

  • Graduate of an accredited Practical Nursing program.

  • Previous Long-Term Care (LTC), Skilled Nursing Facility (SNF), Rehabilitation, or related nursing experience preferred.

Licenses/Certifications:

  • Current Licensed Practical Nurse (LPN) license in the state of practice.

  • Current Basic Life Support (BLS) certification preferred or required according to facility policy.

Essential Functions
  • Provide direct nursing care to residents in accordance with individualized care plans.

  • Administer prescribed medications, treatments, and therapeutic interventions safely and accurately.

  • Monitor, assess, and document resident conditions, reporting significant changes to the Registered Nurse or physician.

  • Assist with admissions, transfers, and discharges.

  • Perform wound care, catheter care, tube feeding assistance, and other nursing procedures within scope of practice.

  • Collaborate with physicians, Registered Nurses, Certified Nursing Assistants (CNAs), therapists, and other members of the interdisciplinary healthcare team.

  • Maintain accurate and timely electronic medical record documentation.

  • Educate residents and families regarding medications, treatments, and healthcare needs.

  • Follow infection prevention, patient safety, HIPAA, and facility policies.

  • Respond appropriately to emergencies and changes in resident condition.

  • Supervise and delegate appropriate tasks to nursing assistants as permitted by state regulations.

  • Perform other job-related duties as assigned.
