About the Role:

As a Speech Language Pathologist at West Park Rehabilitation and Nursing Center, you will be responsible for assessing, diagnosing, and treating a wide range of communication and swallowing disorders in our residents. Your expertise will enable you to develop and implement personalized treatment plans that cater to each resident's unique needs. You will work closely with our interdisciplinary team to provide comprehensive care that addresses the physical, emotional, and spiritual needs of our residents.

Responsibilities:

• Conduct comprehensive assessments to identify communication and swallowing disorders
• Develop and implement personalized treatment plans for residents with varying levels of complexity
• Provide individual and group therapy sessions, including speech, language, and cognitive interventions
• Collaborate with the interdisciplinary team to develop and implement comprehensive care plans
• Monitor and document patient progress, adjusting treatment plans as needed
• Participate in staff training and education programs to share knowledge and best practices
• Maintain accurate and detailed records of patient care and treatment plans
