Clinical Records Coordinator Responsibilities

The Clinical Records Coordinator is responsible for, but not limited to:

  • Initiate and maintain patient clinical records from admission through discharge.

  • Prepare, organize, track, and file all required medical record documentation.

  • Ensure patient charts are complete, accurate, and maintained in accordance with company policies and state and federal regulations.

  • Maintain both electronic and paper medical records while ensuring confidentiality and HIPAA compliance.

  • Track physician orders, certifications, recertifications, plans of care, and other required clinical documents.

  • Coordinate Durable Medical Equipment (DME) orders and communicate equipment issues with clinical leadership.

  • Serve as the primary point of contact for medical record activities between the office, physicians, facilities, and the interdisciplinary team.

  • Follow up with physicians to obtain signed certifications and other required documentation in a timely manner.

  • Retrieve, release, and copy medical records for authorized personnel in accordance with company policies and applicable regulations.

  • Prepare clinical documentation and reports for Interdisciplinary Team (IDT) meetings, including census reports, admissions, discharges, transfers, medication profiles, certifications, and recertifications.

  • Monitor and report patient census information.

  • Participate in daily clinical communication to ensure patient status changes are accurately documented and communicated.

  • Perform routine clinical record audits and promptly identify and resolve missing or incomplete documentation.

  • Support staff in maintaining timely and accurate clinical documentation.

Education
  • High School Diploma required

  • Associate Degree preferred

Qualifications
  • Experience with medical terminology.

  • Experience working with both electronic and paper medical records.

  • Minimum of one (1) year of related healthcare or medical records experience.

  • Strong organizational, communication, and time management skills.

  • Ability to work independently while collaborating effectively with an interdisciplinary team.
