In this role, you’ll help residents regain independence and improve their quality of life through personalized therapy plans. Key Responsibilities:

• Conduct evaluations and create individualized treatment plans tailored to residents’ goals.

• Deliver engaging, evidence-based interventions to promote independence in daily living.

• Monitor and document resident progress, making adjustments as needed.

• Collaborate with a multidisciplinary team to ensure comprehensive, high-quality care.

• Support residents recovering from illness, injury, or disability through rehabilitation services.

• Maintain accurate documentation while safeguarding resident confidentiality.

• Foster a positive, respectful, and compassionate care environment.

Qualifications:

• Graduate of an accredited Occupational Therapy program.

• Active New York Occupational Therapist license.

• Previous experience in long-term care or skilled nursing facilities preferred.
