DESCRIPTION OF WORK

This position provides key support to individuals with developmental delays, mental health needs, or intellectual disabilities. The role focuses on connecting people with essential resources and tracking their progress.

As a County Caseworker 2, you will perform the following duties:

  • Service Monitoring: Responsible for monitoring, linking, and evaluating services. Link to other services and make referrals as needed
  • Direct Contact: Provide regular direct individual contact and provide face-to-face case management services while meeting productivity standards as set for the program
  • Plan Development: Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or family
  • Information Management: Enter necessary information and maintain information in computerized database, state computer systems, and in charts in accordance with state, federal and licensing regulations and Agency policy. Obtain & update information within required time frames
  • Crisis Support: Offer crisis intervention services and coordinate assistance as needed.
  • Collaboration: Work with Agency staff, providers, and community resources to support individuals
  • Transportation Support: Link individuals to transportation services or transport individuals to appointments as needed

In addition, you will attend meetings, staffings, and trainings as needed. You may provide coverage in Somerset County. You will perform other related duties as assigned.


Work Schedule and Additional Information:
  • Full-time employment.
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
    • On occasion, staff may need to work hours outside of the set work week to complete projects, meet work demands or in the event of an emergency.
    • Hours may vary to meet the needs of the consumers.
  • This position reports to the DBHS Office in Somerset for work.
  • Telework: Upon hire, new staff begin training and are on probation for 6 months. New staff may be eligible for a hybrid of on-site and remote work after 3 months of employment.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS
Minimum Experience and Training Requirements:

  • Six months of experience as a County Caseworker 1; or
  • Successful completion of the County Social Casework Intern program; or
  • A bachelor's degree with a social welfare major; or
  • A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.
