Job Description
We are looking for a Home Care Intake Coordinator to support client onboarding, service coordination, and ongoing care planning for individuals receiving in-home services. This contract opportunity begins remotely and is ideal for someone who can balance administrative coordination with compassionate client support while maintaining compliance with Medicaid and program guidelines. The person in this role will help connect clients to appropriate resources, manage active cases, and contribute to a responsive, service-focused care environment.
Responsibilities:
• Manage intake activities from initial referral through service enrollment, ensuring timely setup of in-home care supports.
• Oversee an assigned caseload and maintain regular contact with clients to review changes in needs and update care plans accordingly.
• Coordinate home care services in alignment with federal and state regulations, Medicaid program standards, and contractual obligations.
• Work closely with clients, families, providers, and community partners to support effective service delivery and resolve care-related issues.
• Complete required assessments, documentation, and follow-up tasks, including home visits when needed after onboarding.
• Enter and maintain client and employee information in required web-based systems and program portals with accuracy.
• Support day-to-day operational needs by prioritizing multiple tasks, addressing challenges proactively, and helping maintain an organized workflow.
• Represent the organization professionally at community events, stakeholder meetings, and referral outreach activities to strengthen service awareness.• At least 1 year of experience in home care, intake coordination, benefits coordination, or a related healthcare support role.
• Working knowledge of disability benefits, developmental disabilities services, home health, or home health agency operations.
• Ability to communicate clearly and professionally with clients, referral sources, providers, and internal teams.
• Strong organizational skills with the ability to manage competing priorities and work independently with limited supervision.
• Proficiency with Microsoft Office applications and general computer systems used for documentation and case tracking.
• Experience handling sensitive information while following legal, policy, and regulatory requirements.
• Willingness to conduct in-home visits as required after conversion to a permanent role.
• Bilingual communication skills are preferred but not required.
