American Traveler is hiring an RN for an Outpatient Clinic Care Manager position in Napa, CA, serving a licensed Adult Day Health program with chronic disease management responsibilities.

- Outpatient clinic setting within a licensed Adult Day Health Care (ADHC) program \n
- Serves frail elderly adults, younger disabled individuals, and those with Alzheimer's Disease and related dementias \n
- Program is licensed for up to 100 participants per day with a current average daily census of 72 \n
- Patient ratio of 1:40 \n
- Day shift, 5x8-hour shifts (8:00 AM – 5:00 PM) \n
- No weekend requirements \n
- No on-call requirements \n
- No floating required \n
- Charting is performed in Turbo TAR EMR, which is specific to the CA ADHC industry \n

- Active CA RN license required at time of start; pending CA license accepted \n
- Current BLS certification required \n
- Minimum 1 year of prior RN experience required \n
- Experience in clinic, primary care, pre/post-op, or home health settings preferred \n
- Reliable personal transportation required \n
- Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration \n
- Full SSN and DOB required for consideration \n

- Provides chronic disease case management and monitors medical conditions in collaboration with an interdisciplinary team (IDT) and primary care providers in the community \n
- Delivers caregiver education and support with a program goal of reducing ED visits, hospitalizations, and premature nursing home placement \n
- Local candidates are accepted; candidates residing within 50 miles of the facility are considered local and are eligible at the same rate \n
- Former permanent staff must wait at least 3 months before returning as a traveler and must reside more than 50 miles from the facility \n
- RTO is minimal; a few days may be approved but significant RTO requests may impact consideration \n
- First-time travelers are welcome to apply \n
- Any basic color scrubs are acceptable \n
- Free parking is available on-site \n
