- Facilitates the efficient transfer of patients between facilities. \n
- The Transfer Center Coordinator is a Registered Nurse (RN) responsible for coordinating the safe, timely, and efficient transfer of patients into, out, and within the healthcare system. \n
- This role serves as a central point of communication between referring facilities, physicians, nursing leadership, bed management, and ancillary departments to ensure appropriate placement of patients based on clinical needs, bed availability, and organizational capabilities. \n
- This role ensures compliance with regulations and maintains detailed records for quality assurance. \n
- Previous Epic experience within the last 12 months required. \n

- Night shift: 19:00 - 07:30. \n
\n Schedule Information\n

- Influenza Vaccine: Season Sept. 1 – March 31st. Vaccine or facility declination accepted, due by 12/01 annually. \n
- COVID-19 Vaccine: Declination accepted. \n
- MMR: 2 vaccines or positive titer; if low or equivocal, 1 booster or declination accepted. \n
- Fit Test: Completed prior to start; acceptable masks include Moldex 1510, 3M 1860s, 3M 1860, Moldex 1513, 3M 1870+ Respirator. \n
- Hepatitis B: 2 vaccines or positive titer; if low or equivocal, 1 booster or declination accepted. \n
- TB Test: Proof of negative test within past year; if history of positive test, additional documentation required. \n
- Health Statement: Physician statement within last 12 months. \n
- Driver's License/State ID: Must be current and valid during assignment. \n
- Varicella: 2 vaccines or positive titer; if low or equivocal, 1 booster or declination accepted. \n
- Tdap: Valid for 10 years from administered date; declinations accepted. \n
- GRH Background Check: Within 30 days of start; includes SS trace and county search for last 7 years. \n
- GRH Iowa Adult and Child Abuse: State requirement. \n
- GRH Education Verification: Verified through 3rd party; combine with diploma/transcripts if needed. \n
- GRH EPLS/GSA/SAM HHS/OIG: Ran within 30 days of start. \n
- GRH References: 2 professional references required, 1 must be from supervisor. \n
- GRH Drug Screen: Within 30 days of start; 9 panel excluding THC; instant accepted if results unavailable. \n
- GRH Skills Checklist: Complete within 12 months; does not expire. \n
- GRH SING: Ran within 30 days of start. \n
- GRH Resume: Required. \n
- GRH I9 Attestation: Required on agency letterhead. \n
- GRH Color Vision: Required. \n
- GRH Contract Employee Data Sheet: Required. \n
- GRH NSO: Ran within 30 days of start; upload if ran with background check. \n
- GRH OFAC: Ran within 30 days of start; upload if ran with background check. \n
