  • Facilitates the efficient transfer of patients between facilities.
  • \n
  • The Transfer Center Coordinator is a Registered Nurse (RN) responsible for coordinating the safe, timely, and efficient transfer of patients into, out, and within the healthcare system.
  • \n
  • This role serves as a central point of communication between referring facilities, physicians, nursing leadership, bed management, and ancillary departments to ensure appropriate placement of patients based on clinical needs, bed availability, and organizational capabilities.
  • \n
  • This role ensures compliance with regulations and maintains detailed records for quality assurance.

  • Previous Epic experience within the last 12 months required.
