  • Provides each Elder with routine daily nursing care and other desired services in accordance with the Elder’s assessment, choices and care plan.
    • \n
  • Works closely with the RN Manager to manage the day-to-day functions.
    • \n
  • Facility is a total of 160 beds.
    • \n
  • Resident to Nurse ratios will be discussed during the interview.
    • \n
  • Previous Epic experience within the last 12 months in a LTC setting required.
    • \n
  • This position will have variable shifts with most shifts being on either Evenings (1430 - 2300) or Nights (2245 - 0645).

  • Must have BLS and ACLS certifications.
    • \n
  • Previous Epic experience within the last 12 months in a LTC setting required.
    • \n
  • Primary Source Verification of professional license required within 30 days of start (for RNs use BON & NURSYS).
    • \n
  • Must have valid professional license/certification as required for the position.
    • \n
  • Completion of GRH Skills Checklist within 12 months required.
    • \n
  • Pre-Orientation CBL Acknowledgement required.
    • \n
  • Influenza Vaccine or Facility declination accepted, due by 12/01 annually (optional).
    • \n
  • COVID-19 Vaccine or declination accepted.
    • \n
  • MMR: 2 vaccines or positive titer; if low or equivocal 1 Booster or declination accepted.
    • \n
  • Fit Test completed prior to start with acceptable masks.
    • \n
  • Hepatitis B: 2 vaccines or positive titer; if low or equivocal 1 Booster or declination accepted.
    • \n
  • TB Test proof of negative test completed in past 2 years or documentation if prior positive test.
    • \n
  • Health Statement from physician within last 12 months, updated every 4 years.
    • \n
  • Current Driver's License/State ID required.
    • \n
  • Varicella: 2 vaccines or positive titer; if low or equivocal 1 Booster or declination accepted.
    • \n
  • Tdap valid for 10 years from administered date; declinations accepted.
    • \n
  • Background Check within 30 days of start including SS trace and county search for last 7 years.
    • \n
  • Iowa Adult and Child Abuse state requirement.
    • \n
  • EPLS/GSA/SAM HHS/OIG check within 30 days of start.
    • \n
  • Two professional references required, one must be from supervisor.
    • \n
  • Resume or profile submission required.
    • \n
  • I-9 Attestation required.
    • \n
  • Color Vision test proof within 1 year.
    • \n
  • NSO and OFAC checks within 30 days of start.
    • \n
  • Evergreen Link acknowledgment must be completed within 48 hours of offer acceptance.
