Discharge Planner Job Summary:

Discharge Planner Responsibilities:

• Coordinate with multidisciplinary teams, including physicians, nurses, and social workers, to develop comprehensive discharge plans for patients

• Conduct patient assessments and identify barriers to discharge, developing strategies to overcome these barriers

• Facilitate communication between patients, families, and healthcare providers to ensure a smooth transition to the next level of care

• Monitor patient progress, adjusting discharge plans as needed to ensure optimal outcomes

• Identify and address potential discharge delays, implementing corrective actions to maintain efficient discharge processes

• Maintain accurate and timely documentation of discharge plans, patient progress, and coordination efforts

Requirements:

• Prior experience in discharge planning or a related field preferred

• Degree in Social Services or related field a plus

• Strong communication, organizational, and problem-solving skills

• Ability to work in a fast-paced environment, prioritizing multiple tasks and deadlines

• Proficiency in electronic medical records and discharge planning software

• Strong attention to detail, with excellent documentation and record-keeping skills

• Ability to maintain confidentiality and handle sensitive information with discretion
