Hudsonwide Health Care is looking for an energetic, relationship-driven Admissions Director to join our 120-bed Skilled Nursing Facility in Hyde Park, NY. Salary: Commensurate with experience

• Lead and coordinate the full admissions process—from referral to intake to approval—ensuring a seamless and timely experience
• Build and strengthen relationships with hospitals, physicians, and community partners to drive consistent referrals
• Serve as the admissions expert, maintaining compliance with all policies, procedures, and documentation requirements
• Champion the facility’s brand through marketing efforts and community outreach
• Navigate Medicare, Medicaid, and Managed Care processes with confidence
• Collaborate with the care team to ensure smooth and successful resident transitions
• Host tours, presentations, and family meetings to showcase our facility and services

• Proven experience as an Admissions Director or Admissions Coordinator in post-acute or long-term care
• Strong knowledge of local hospital systems and referral networks
• Excellent communication, organizational, and time-management skills
• A proactive, results-oriented mindset with the ability to manage multiple priorities
• Comfort working both independently and as part of a collaborative team
• Proficiency in MS Office (experience with Sigma, NAVI, and Allscripts is a plus)
