Here’s What You’ll Do for Our Residents & Facility:

• Supervises Staff: Manages, hires, trains, and schedules social work staff and interns.

• Administrative: Establishes departmental policies, procedures, and quality improvement standards & oversees the social services department budget and resource allocation.

• Enforces Regulations: Ensures the facility complies with federal (CMS), state, and local social service mandates & Serves as the primary department contact during state health inspections and surveys

• Maintains Documentation: Maintains & audits medical charts to ensure psychosocial documentation meets strict legal standards.

• Collaboration: Participates in care planning and other meetings, including Quality Assurance and Performance Improvement (QAPI).

• Care & Mediation: Steps in resolving severe, complex family grievances or resident crises. Provides supportive care as needed to supplement social work needs

• Community Liaison: Builds relationships with local hospitals, mental health organizations, and community resources to streamline admissions and discharges.

Does This Sound Like You?

• Licensed to work within the State of Connecticut as a LSW Licensed Social Worker

• Able to pass background check, drug screen, professional references check & physical evaluation

• High level understanding of leadership, customer service and clinical competency

• Leadership experience in a long-term care setting

• Prior experience as a Director of Social Services/Social Work Director
