Job Summary:

Cedar Healthcare Center seeking a compassionate and experienced Director of Social Services to join our team in Portsmouth, New Hampshire. As a key member of our leadership team, you will oversee the development, implementation, and evaluation of our social services programs, ensuring that our patients receive the highest quality care and support.

Responsibilities:

• Develop and implement comprehensive social services plans that address the unique needs of our patients, including but not limited to counseling, discharge planning, and community resources

• Collaborate with interdisciplinary teams to develop and implement patient care plans that address social, emotional, and psychological needs

• Conduct assessments and evaluations to ensure that patients are receiving the appropriate level of care and services

• Develop and maintain relationships with community organizations and resources to ensure access to necessary services for our patients

• Stay up-to-date on relevant laws, regulations, and industry standards to ensure compliance and quality of care

• Participate in quality improvement initiatives to continuously improve our social services programs and outcomes

Requirements:

• Master's degree in Social Work or related field

• Minimum 2 years of experience in a leadership role or social services setting

• Strong knowledge of social services principles, practices, and regulations

• Excellent leadership, communication, and interpersonal skills

• Ability to work effectively in a fast-paced environment with multiple priorities

• Strong problem-solving and analytical skills
