Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for Element Care PACE in Brighton, Massachusetts.

POSITION OVERVIEW
• Provide comprehensive primary care for older adults enrolled in the PACE program
• Evaluate, diagnose, and manage acute and chronic medical conditions in an outpatient setting
• Perform routine follow-up care while supporting continuity through occasional community-based visits
• Collaborate with physicians, nurses, rehabilitation staff, social workers, and the interdisciplinary PACE team
• Build long-term relationships while helping seniors remain healthy, independent, and safely living in the community

COMPENSATION
• $106,000-$149,000 Annual Salary

SCHEDULE
• Monday-Friday
• 8:00 AM-4:00 PM
• No Nights
• No Weekends
• No On-Call Responsibilities

• Active Massachusetts Nurse Practitioner or Physician Assistant License REQUIRED
• Current FNP, AGNP, or PA-C Certification REQUIRED
• Experience Managing Acute and Chronic Primary Care Conditions REQUIRED
• Minimum One Year of Clinical Experience Preferred
• Strong Clinical Assessment and Patient Management Skills
• Commitment to Team-Based Care for Older Adult Populations
• Comfortable Providing Occasional Community-Based Patient Visits
