We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

As a Nurse Practitioner with Carease Health, you will have the opportunity to make a meaningful impact on patient care while enjoying a flexible part-time schedule. Your responsibilities will include:

• Providing high-quality preventative care to residents in skilled nursing facilities

• Conducting on-site visits and remote work from home

• Collaborating with healthcare teams to develop and implement comprehensive care plans

• Utilizing remote patient monitoring, chronic care management, and psychiatric collaborative care to optimize patient outcomes

• Maintaining accurate and timely documentation

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and Documentation review

• Excellent communication, collaboration, and teamwork skills

• Strong knowledge of Geriatric and Internal Medicine principles
