Social Worker

A skilled nursing facility is seeking a compassionate and experienced Social Worker to join our team! This is an excellent opportunity for someone who is passionate about resident advocacy, discharge planning, and supporting residents and families through every stage of care.

Responsibilities:

• Assist residents and families with emotional, social, and financial needs
• Coordinate discharge planning and transitions of care
• Complete assessments and maintain accurate documentation
• Collaborate with interdisciplinary team members to ensure quality resident care
• Advocate for resident rights and overall well-being
• Provide support during admissions, care conferences, and family meetings
• Maintain compliance with state and federal regulations

Qualifications:

• Social Work experience in a skilled nursing facility or healthcare setting preferred
• Knowledge of Medicaid, Medicare, and discharge planning processes
• Strong communication and interpersonal skills
• Ability to multitask and work in a fast-paced environment
• LMSW or related credentials preferred, but not required depending on experience
