Social Worker

Foothill Acres Nursing & Rehab Center is looking for a full time Social Worker in our 200 bed LTC/SAR facility. Main responsibilities include care plan meetings and discharge planning coordination, conducting MDS assessments, and addressing the psycho-social needs of the patients. The ideal candidate will be caring and compassionate and able to work as part of an interdisciplinary team. LSW and one year experience in a SAR/Long Term Care setting preferred.
