Located in Queens, NY

Salary: $85K to $95K Range; based on experience 

Requirements:

• Must have prior nursing home / long-term care experience
• Previous experience as a Social Work Director 
• Strong knowledge of discharge planning, care coordination, and resident advocacy
• Experience working with interdisciplinary teams
• Familiarity with state and federal regulations related to long-term care
• LMSW strongly preferred, but not required

Job Description:

• Plans and directs a comprehensive social services program which provides support services for residents and families.
• Responsible for coordinating all admissions and discharges to ensure facility census is kept at its maximum capacity.
• Participates in an interdisciplinary team approach to better serve the needs of the facility population.
• Oversee and manage the Social Services department
