Job Summary
As a Social Worker at Vantage at West Springfield, you will provide psychosocial support to residents and families while assisting with care planning and discharge coordination. This role plays an essential part in advocating for resident needs and supporting successful transitions.

Responsibilities

·        Conduct psychosocial assessments

·        Participate in care planning

·        Support residents and families

·        Coordinate discharge planning

·        Maintain documentation and compliance

Pay Range: $70,000/year - $85,000/year
