Care Team Coordinator-BEACON, NY

We are looking for a dependable and patient-focused Care Team Coordinator to become part of our healthcare team in Beacon, NY. This role combines front-desk coordination, patient communication, and clinical support responsibilities to help keep daily operations running smoothly.

What You’ll Be Doing:

• Coordinate patient visits and maintain provider schedules
• Answer incoming calls and assist patients with questions and appointment needs
• Help facilitate communication between patients, providers, pharmacies, and insurance companies
• Obtain and update patient demographic and insurance information
• Support clinical staff with basic patient intake duties, including obtaining vital signs
• Enter and maintain accurate patient documentation within the electronic charting system
• Assist with general administrative and operational tasks throughout the office

What We’re Looking For:

• Prior experience working in a medical practice or healthcare office environment
• Background in patient coordination, front desk healthcare support, medical assisting, or similar roles preferred
• Comfortable using electronic health record systems, preferable eClinicalWorks (eCW)
• Strong organizational and communication skills
• Ability to prioritize tasks and remain professional in a busy setting
• Team-oriented attitude with a strong focus on patient service

Position Details:

• Full-time schedule, M-F, 9:30 AM - 5:30 PM
• Monday through Friday
• Daytime hours only
• No overnight or weekend shifts
