Essential Role:
Provide care and support to residents by following individualized assistance plans.

• Pay Rate: $19.15 an hour

• Shifts: Full-time and part-time opportunities available

• Assist residents with personal care activities

• Support with light housekeeping and daily tasks

• Ensure resident safety, including proper use of equipment (wheelchairs, oxygen, side rails, etc.)

• Accurately document services in accordance with agency policies

• Communicate any changes in a resident’s condition to the RN Supervisor promptly

• Collaborate with the healthcare team as required by NYS regulations

• Maintain confidentiality of resident and family information

• Build positive relationships with residents and their families

• Follow dress code and maintain professional hygiene standards

• Demonstrate reliability, punctuality, and professionalism

• Report concerns regarding resident well-being to the Case Manager or Administration

• Perform additional duties as assigned by the supervising nurse
