Position Summary

The Care Coordinator is a unique hybrid role that combines care coordination, caregiver support, field leadership, and exceptional client service to ensure the best experience for clients and their families.

Key Responsibilities

• Manage client files, documentation, and insurance verification.

• Coordinate with insurance providers and referral sources.

• Conduct client intakes, follow-up calls, and satisfaction visits.

• Support, train, and mentor new caregivers.

• Follow up on leads and prospective clients.

• Start new cases and coordinate transitions to caregivers.

• Provide coverage for caregiver call-offs as needed.

• Serve as a liaison between clients, families, caregivers, and the office team.

• Help maintain high standards of care and client satisfaction.
  

Qualifications

• Compassionate, organized, and detail-oriented.

• Strong communication and problem-solving skills.

• Comfortable working in both office and field settings.

• Bilingual in English and Spanish preferred.

• Reliable, professional, and adaptable.

• Focused on delivering exceptional client service and quality care.
  

Position Details

• Employment Type: Full-Time

• Starting Pay: $21.00 per hour

• Work Environment: Hybrid (Field & Office-Based)
