Director of Social Work

Hudson Wide is seeking an experienced and compassionate Director of Social Work to join our team. This leadership role is ideal for a motivated professional who is dedicated to enhancing the quality of life for our residents and supporting families through every stage of care.

Responsibilities:

• Conduct psychosocial assessments for new admissions

• Develop and implement individualized care plans

• Coordinate interdisciplinary care planning and discharge planning

• Complete timely documentation, including admissions, quarterly assessments, and MDS

• Investigate and document resident concerns and grievances

• Serve as a liaison between residents, families, and the care team

Qualifications:

• Bachelor’s or Master’s Degree in Social Work or related field

• Healthcare experience required; skilled nursing experience preferred

• Social Work licensure preferred

• Strong communication, organizational, and interpersonal skills
