The Opportunity:

PERIOD OF PERFORMANCE

ONE YEAR – twelve (12) months from the date of the award plus four option years

WORK SCHEDULE

A. The Contractor shall provide on-site pharmacist services and coordinate patient care with all appropriate

departments.

B. Normal shifts for the Pharmacy Department consist of 8 hour shifts. Working hours are shifts between the

hours of 8:00 am to 5:00 pm with a 1 hour lunch break or 8:00 to 4:30 pm with a half hour lunch break depending

on the site assigned, Monday through Friday (7:00 am to 4:00 pm, 9:00 am to 6:00 pm or 1:00 pm to 10:00 pm)

and 10:00 am to 6:00 pm Saturday and Sunday (at select facilities). No federal holidays are worked.

PROFESSIONAL QUALIFICATIONS

A. Graduate of accredited College of Pharmacy

B. Documentation of annual Continuing Pharmacy Education required for licensure

C. Shall possess current unrestricted license to practice in a state, the District of Columbia, the Commonwealth of

Puerto Rico, or a territory of the United States at all times

D. Contractor shall submit resume of past professional experience

E. Documentation of a Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS) from

the Board of Pharmacy specialties OR completion of a Post Graduate Year One residency

F. Demonstrated experience in management of patients with diabetes within the context of a collaborative practice

agreement.
